• Pick up from hospital or rehab

• Transportation to the client’s residence

• Pick up prescriptions from the pharmacy

• Ensuring that the fridge has fresh food (grocery run)

• Help to unpack and explain the discharge instructions

• Settle clients and make sure they are safe and comfortable

• Provide post-acute in-home care until back to being independent
